ADMISSION PROCESS

To be considered for admission to Woodstock Manor, please submit the following via fax or email:

✔︎  A completed Application for Admission form

✔︎  Psychosocial history

✔︎  Most recent psychiatric assessment

✔︎  Current comprehensive treatment plan and/or discharge plan

Woodstock Manor's Administrator will conduct an Admission Interview and Assessment and review all supporting documentation to determine whether Woodstock Manor may be an appropriate placement. All documents must be submitted prior to admission.
